Meet the Staff of Access HealthCare
David M. Smith, M.D.
Dr. Smith was born in 1948. He grew up on the Lake Champlain Islands of Vermont and graduated from Duke University in 1970.
Dr. Smith is a veteran of the the U.S. Navy. He obtained his medical degree from the Medical College of Virginia in 1978 and completed his Residency training at Roanoke Memorial Hospital in 1981.

Dr. Smith has been married since 1973 and has three grown children. He has practiced as a board-certified family physician in Forest since 1981. He founded Access HealthCare in 2000.
Manuel T. Peralta, Jr., M.D.
Dr. Peralta was born in the Philippines in 1972 and completed his medical degree from the University of East Ramon Magsaysay Medical Center in 1999. Dr. Peralta completed his Residency training at the Carilion Family Medicine Residency from 2004 through 2007. He became board certified as a family physician in 2007 and joined Access HealthCare in the summer of 2007.

Dr Peralta’s wife Nathalie, also from the Philippines, is a Physical Therapist. The Peraltas have three small children.
