Access HealthCare is Accepting New Patients
If you want to become a patient at Access HealthCare, please contact us to begin the enrollment process.
Our office staff will review your insurance coverage and discuss our office policy so there will be no confusion regarding insurance or other financial concerns.
In advance of your first appointment, you will need to download and complete several of our office forms, which can be dowloaded in either Microsoft Word or Adobe PDf format.
Bring the completed forms to your first visit here so we can review and fully discuss the information with you.
First appointments with our physicians will be scheduled as soon as possible, but may take a few weeks depending on the circumstances. We will do everything we can to meet your needs in addressing more urgent health problems.
